Plan, Develop and Succeed

Through a 3-phased process we call Plan, Develop and Succeed, Resolve 16+ Support Ltd partners with young people, who are transitioning out of ‘Leaving Care’ to experience a life marked by safety and purpose. Every resource offered by our team is aimed at developing the young person’s self esteem and worth, confidence and resilience in order that they might experience successful independent living in the community.

Plan
During this first phase, young people learn the basics of what it takes to live independently successfully. Following an assessment interview, the new resident begins working closely with a member of the team to develop a comprehensive transitional plan that includes short and long-term goals.

Our holistic package of support is designed to empower young people to plan how to live successful and independent lives.  Our team will never undertake a task for a young person if they can do it for themselves. This is because we believe it is important for young people to get as much hands-on experiences as possible as they work toward accomplishing their goals.

There are many aspects of our programme which help young people plan towards successful adulthood. Residents during this phase live in semi-supervised housing to develop their community and life skills such as cooking, cleaning, laundry and home maintenance. They will be offered opportunities to complete education programmes and take steps toward obtaining qualifications or vocational training. Establishing a support network of peers, adults and learning career skills and obtaining advice for employment are key goals. Our plan is to assist our young people to recognise that they can achieve numerous successes in their lives.

Develop
During the Develop phase, our team helps young people to stay on a track to achieve their goals. As our young people progress toward successful adulthood, they take on more responsibilities and enter the community with a greater sense of independence. The focus in this phase is for each young person to establish, enhance and reinvigorate their support network. The young people residing in semi-independent property will have access to supportive team on a regular basis.

During this phase, the young person’s reliance on the team is slowly fading as they become autonomous with their personal network of support and the community. Young people in this phase have a greater freedom in choosing how they will participate in the programme. Nonetheless, the team’s focus during this phase is based on promoting opportunities for the young people. For most young people in this stage, career and educational development remain key components of their everyday life. Near the completion of this phase, young people are now moving closer to fulfilling their transitional plan.

Succeed
Full independence and success are all within reach during this last phase. The lives of young people are marked by attendance to work training programmes, enrolment in college or other vocational courses. Young people in this final phase of their transition demonstrate a desire to live harmoniously.

During the Succeed phase, young people will be aiming to secure their own tenancy. They will be expected to keep their own appointments for health care, mental health, college classes and work schedules. By this time, young people will have developed adequate life skills necessary for successful independence.

Still, we recognise that young people might encounter difficulties and therefore having an active outreach worker is important. We expect young people during this phase to be living independently whilst having the knowledge that they have someone who can offer support when life gets tough. That is success.
